Western medicine treatment of acute thyroiditis:

Cold compresses should be used locally in the early stage, hot compresses should be used in the late stage, and antibiotics should be given systemically. When there is an abscess, it should be incised and drained as soon as possible to prevent the abscess from damaging the trachea, esophagus, and mediastinum. Treatment includes the use of antibiotics and local incision and drainage. It is best to perform puncture fluid and blood culture, and perform drug sensitivity tests, and then select sensitive antibiotics to more effectively eliminate bacteria. After a few days of abscess formation, the thyroid abscess must be incised and drained to effectively remove the pus and control local inflammation as soon as possible. Since the neck pain is obvious, some pain medications such as pain tablets and salicylic acid preparations can be used.

1. Internal treatment

1. Wind-heat phlegm condenses and causes pain and red burning sensation in the neck; accompanied by chills, high fever, headache, and dry throat; the tongue coating is thin and yellow, and the pulse is floating or slippery.

Syndrome differentiation and analysis: Wind-heat invades the lung and stomach, burning body fluid into phlegm, accumulated heat causing phlegm, sore throat, and lumps in the neck; wind-heat condenses into phlegm, obstructing the meridians, poor circulation of qi and blood, and causing lumps and pain; toxic heat is strong, so symptoms of heat such as chills, high fever, dry throat, and slippery pulse are seen; thin yellow tongue coating and floating and rapid pulse indicate wind-heat blockage.

Treatment: Soothe wind, clear heat and resolve phlegm.

2. Due to qi stagnation and phlegm stagnation, there is a solid lump in the front of the neck, with mild swelling and pain. When the lump is pressed, the pain is reflected to the back of the head; sometimes accompanied by laryngeal obstruction; the tongue coating is yellow and greasy, and the pulse is stringy and slippery.

Syndrome differentiation and analysis: Liver depression and stomach heat, burning body fluid turns into phlegm, qi stagnation and phlegm coagulation, throat congestion, so the lump in front of the neck is firm, swollen and painful; the swelling is severe, so there is a feeling of obstruction in the throat; the tongue coating is yellow and greasy, and the pulse is stringy and slippery, which indicates phlegm heat.

Treatment: Soothe the liver and clear away heat, resolve phlegm and disperse nodules.

2. External Therapy

In the initial stage, you should use medicine such as Jinhuang Powder, Sihuang Powder, Shuangbai Powder, Yuluxu Powder, mixed with cold boiled water or honey into a paste for external application 1-2 times a day.

In the second abscess stage, if the mass has obvious fluctuations, it can be drained by incision or puncture.

After the pus is cleared during the third healing period, Shengji Powder can be applied externally to promote wound healing.
